Responsibilities
- Assists with phone calls to and from field staff, physician offices, referral sources, patients, and residential and healthcare facilities. Takes messages and receives physician verbal orders, as needed. Fields all phone calls and completes all follow up in a timely manner.
- Frequently monitors the status of incoming faxes, following up with the appropriate person, as needed.
- Maintains competency through assigned online courses and required in-services.
- Assists with visit verification, scheduling, and projection of visits, according to visit frequency orders and
- Maintain up to date knowledge, understanding, and skills related to the electronic medical record documentation system, all patient care system applications, and hospice agency operations applications such as Outlook, Teams, learning management system, and policy and procedure manuals
Education and Experience
License Requirements
- Current LPN/LVN license in state of practice .
- Current CPR certification required.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
Additional State Specific Requirements
- LA: Minimum of two years full time experience working as an LPN.
