Overview:

Per Diem RN Responsible for providing skilled nursing care to patients of the Mobile Outreach Clinic Program

Responsibilities:

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Provides case management for program clients
• Monitors and ensures quality and appropriateness of services provided are in accordance with the agency and contract guidelines
• Provides completed documentation in accordance with agency guidelines
• Develops and provides individual and group education for patients and site staff
• Establishes and maintains community relationships to support client referrals
• Adheres to agency infection control and safety policies
• Adheres to the organization’s policy in regards to absenteeism and appearance.

Qualifications:

RN licensure for New Jersey

2+ years of community health nursing experience; bilingual preferred

Strong Computer skills

Excellent verbal and written communication skills

Excellent interpersonal skills with demonstrated ability to communicate with outside personnel

Ability to travel with a valid NJ driver’s license required
