Overview:
<p>Responsible for providing comprehensive nursing services to women and their families eligible for the Nurse Family Partnership program in Ocean County.  </p>

Responsibilities:
<li>Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers</li><li>Attends weekly team meetings, case conferences and clinical supervision meetings</li><li>Assesses and evaluates needs of women and their families as they relate to health and life course development</li><li>Carries a caseload of up to 25 clients and completes home visitation in accordance with the NFP model elements and NFP Visit-to-Visit guidelines</li><li>Provides counseling and instruction to assist women and their families to attain targeted goals</li><li>Develops and maintains community relationships to support client referrals </li><li>Precepts and mentors new hires and makes visits with student nurses as needed</li><li>Adheres to agency infection control and safety policies</li><li>Adheres to the organization’s policy in regards to absenteeism and appearance</li>

Qualifications:
<li>Bachelor’s degree in Nursing </li><li>2-3 years work related experience</li><li>Strong computer skills  </li><li>Excellent verbal and written communication skills </li><li>Excellent interpersonal and organizational skills</li><li>Bilingual- Spanish speaking a plus</li>
