Immediate opening for an LMSW Case Manager to work in a large social services agency in the Bronx! The schedule for this position is Monday through Friday, 9am -5pm. 

Job Function:

• Assist clients to locate an alternative housing option when a client’s health so requires
• Provide delivery of mental health and alcohol/substance abuse services and referrals
• Assesses clients and/or family psychosocial status, social work needs and living conditions utilizing professional knowledge, skills of observations and interviewing skills.
• Establishes the social work component of the patient/family plan of care based on goals mutually acceptable to the client, family and significant others. Makes referrals to other community services, as necessary.
• Provides psychosocial work services to patient and/or family, including short-term individual counseling, community resource planning, and crisis intervention.  

Job Requirements:

• Bachelor's Degree in Social Work, Public Administration preferred. Associates degree with at least 4
  years of related experience or High School degree with at least 6 years of case management experience
  may be accepted
• Individual and group counseling experience with mental health patient population
• 1 yr experience with homeless population  
• 1 yr   experience with persons with chronic health and/or mental health conditions and/or substance abuse disorders and geriatrics
• Bilingual in Spanish preferred
