WCS Healthcare has an immediate opening for an RN Case Manager with a large Social Services Agency with needs in Suffolk County.

Job Function:

• Day Shift
• 8:30am-5pm flexible on start time
• Assess individual's needs.
• Build strong relationships with individuals, families, and the treatment team.
• Create and ensure support staff stick to nursing care plans.
• Schedule appointments for individuals promptly.
• Teach individuals and support staff regarding nursing plans of care required.
• Document individuals status, health and follow-ups.
• Monitor health records such as seizure charts, bowel movement tracking, vitals, etc.
• Manage medication storage properly, dispose of expired medications correctly, and keep the medication area clean.

Job Requirements:

• Registered Professional Nurse with licensure from New York State Education Department
• 1 year nursing experience working with Intellectual Disabled Adults or Case Management experience preferred
