Description
Our client, one of the largest non-profit OPWDD organizations in New York, specializing in care for IDD adult patients, is seeking an RN Case Manager in the Bronx. This is a hands-off nursing position where the candidate will be directing how to care for the patients and not doing the hands-on care work themselves.
Job Function:
- Day Shift
- 9am - 5pm
- Oversee direct support professionals in various home-care settings. Settings range from small houses to apartment complexes.
- Responsible for about 2 houses with about 5-10 patients in each, at most.
- Expected to float between houses each day of the week as per required by patients' needs but will only need to report to one home each day. They will not need to float among multiple houses in a single day.
- Responsible for documentation on a regular basis.
- You are able to start/stop their day for personal reasons if/when necessary, as well as setting their own schedule on a weekly basis, within reasonable day-time hours.
Job Requirements:
- New York State RN License
- Willingness to work with IDD population and be flexible with scheduling
- OPWDD or Case Management experience required
