Description
A very well known social services organization is looking for a part or full time LPN Case Manager to join their growing team in Bronx, NY!
Job Function:
- Day Shift
- 9am - 5pm
- Provides direct and supportive healthcare to individuals that reside in IAHD's residential programs
- The position focuses on coordinator of care/case management for individuals and staff training oversight for the provision of care
- Documenting individual's status, health needs and follow-up
- Attending staff meetings and participating in company functions as needed
Job Requirements:
- New York State LPN License
- Experience in direct clinical work with developmentally disabled populations preferred
