Description

Our client, one of the largest non-profit OPWDD organizations in New York, is looking for a Nurse Case Manager in Brooklyn, NY. This is a day shift position, 9am - 5pm. The weekly pay for this position is $1,600 - $1,800.


Job Function:

  • Oversee direct support professionals in various home-care settings. Settings range from small houses to apartment complexes.
  • Responsible for about 2 houses with about 5-10 patients in each, at most.
  • Expected to float between houses each day of the week as per required by patients' needs, but will only need to report to one home each day. They will not need to float among multiple house in a single day.
  • Directing care for the patients and managing documentation of care on a regular basis.  

Job Requirements:

  • New York State RN License  
  • Willingness to work with IDD population and be flexible with scheduling
