RN Care Coordinator - OBGYN- Days


Overview JOB DUTIES Under the supervision of the Administrative Manager, provides coordination of care and patient education for Maternal Fetal Medicine patients in the in-patient and out-patient setting. Assists physicians in the provision of quality health care. Coordinates high-risk obstetrical patient transfers from referring physicians. Essential Functions • Arrange for appointments (new and follow-up) for patients referred to the MFM service and patients who need follow-up after surgery or other treatment. This would include facilitating authorization for these appointments. • Triage calls for Maternal Fetal Medicine patients (Collects and assesses patient data to determine patient needs and to promote optimal patient care); Document calls in Epic (when available for outpatients). Call in prescriptions to pharmacies as directed by members of the MFM team. • Coordinates care of MFM current and new obstetrical patients. Assumes responsibility and accountability for tracking lab, pathology, cytology and radiology procedures ordered to ensure results are received and viewed by the responsible provider. Reviews normal test results with patients. Discusses non-routine results with MFM doctor(s) prior to reviewing with the patients. • Collects and assesses patient data to determine patient needs and to promote optimal care. Coordinate transfer of patient records. Calls and follows up with patients after surgery. • Responsible to the Director, Division of Maternal Fetal Medicine for providing complete interdisciplinary management of nursing services for individuals seen by the Maternal Fetal Medicine providers. • Coordinates education and guidance for the patient and significant others to maximize the achievement of the expected outcomes. • Plans and implements age-appropriate care based on assessment, diagnosis, standard of care, medical treatment plan, and progress toward desired outcome. Assesses individual physical, psychosocial and developmental needs of patients. • Assumes responsibility and accountability to ensure appropriate referrals are made for special procedures. This would include facilitating authorization for these procedures. Temperament Must be able to perform under stress when confronted with emergency, critical, or unusual situations. Must be capable of dealing with periodic cyclical workload pressures and levels of responsibility. Required to make independent judgments without supervision. Must be able to make generalizations, evaluations, or decisions based on sensory or judgmental criteria. Must have the adaptability to perform a variety of duties, often changing from one task to another of a different nature without loss of efficiency or composure. Requires the ability to work with people beyond giving and receiving instructions. Qualifications SKILLS, QUALIFICATIONS, AND REQUIRED EXPERIENCE Skills, Knowledge, Abilities: Regulations/Policies • Knowledge of regulatory agency standards (e.g. Joint Commission, HRS), Nursing Standards and Florida Nurse Practice Act. Training/Support • Demonstrated Clinical skill in area of ambulatory care and/or particular specialization (e.g., Pediatrics, ENT). Office Management • Basic knowledge of management practices and standard supervisory protocols. Clinical Duties • Ability to serve as a resource for the nursing process. Medical Terminology • Knowledge of medical procedures and medical terminology. Professionalism • Strong interpersonal skills including a professional and positive demeanor. Regulations/Policies • Ability to interpret and implement policies and procedures. Experience Requirements: 3 years Minimum of three years nursing experience required. Up to 1 year Experience in an ambulatory care setting in the field of Obstetrics and Gynecology preferred. Up to 1 year knowledge of regulatory agency standards (e.g., JCAHO, HRS), Nursing Standards and Florida Nurse Practice Act required Education Requirements Associate's degree in nursing is required Bachelors Nursing preferred Certification/Licensure Requirements Registered Nurse (RN) - Florida Licensure required at time of hire Basic Life Support (BLS) required at time of hire UFJPI is an Equal Opportunity Employer and Drug Free Workplace
