Sign-On Bonus Details: * $6,000 sign-on bonus available for nurses with over 1 year of experience The General Internal Medicine, Oakland team based in Montefiore Hospital is hiring a Full time OP Nurse Coordinator II to join their clinical leadership team. This position will be responsible for supervising the flow of the daily clinic under the direction of the Sr. Practice Manager, managing clinic RN's and Medical Assistants, training/hiring clinic staff, and ensuring compliance with outpatient clinic regulations. Ideal candidate will have previous Outpatient Clinic experience and minimum of 1-year supervisory experience. Schedule: * Monday-Friday * Daylight hours Purpose: The Professional Practice Coordinator is a Registered Nurse and is a member of the care delivery team. The Professional Practice Coordinator is responsible for the care of the patient in a global view, accountable for multiple aspects of the patient care package even when patient contact is minimized. The Professional Nurse Coordinator uses independent judgment and continued communication with the patient, family, physician and community resources to assist the patient in attaining a measure of outpatient health. The Professional Practice Coordinator is responsible to set the standards for the level and quality of care. The Professional Practice Coordinator maintains daily accountability for the clinical and administrative operations of the assigned practice. The Professional Practice Coordinator assures overall efficiency and quality care to the patients served. The Professional Practice Coordinator demonstrates a commitment to the community and to the nursing profession. Responsibilities: * Identify potential problem areas, develop a system for objectively monitoring performance, and creatively seek solutions to foster quality improvement. Actively manages all aspects of regulatory compliance, including all outside agencies and audits as well as internal requirements, such as core competencies and training. Actively manages department or practice-specific quality improvement efforts. Identifies opportunity for quality improvement to colleagues and management. Takes personal responsibility in improving patient satisfaction with the quality of care and service. Utilizes research and evidence-based practice to support improvement in clinical care: identifies research issues or articles related to clinical specialty or areas of interest * Demonstrates the ability to create a shared vision that applies critical thinking skills and utilizes conceptual knowledge in the development of the departmental vision. Demonstrates accountability for professional development that improves the quality of professional practice and the quality of patient care. Establishes expectations and model behaviors that demonstrate service excellence to staff and physicians and focuses on the patient as the primary customer. Functions as a leader in practice-based shared governance, goal setting and supports the change and transition process to improve quality of care and the practice environment. * Demonstrates critical thinking in the identification of clinical, social, safety, psychological and spiritual issues for the patient care within an episode of care. Practices solid communication skills and is able to articulate and translate the patient's condition to other care providers and to negotiate and make recommendations for changes in patient care and office practices. Patient documentation is comprehensive and promotes communication between caregivers. Supervisory/leadership ability to evaluate staff, address issues appropriately and continually monitor and improve clinical services provided. Accountable for managing the interviewing, selection, hiring, training, evaluating, and progressive discipline process of staff. Demonstrates knowledge of adult learning principles (and/or teaching children if applicable) and applies in teaching of patients, families, students and new staff. Provides detailed and appropriate teaching to patients and families to effectively guide them through the episode of care both in the office setting and via phone/written communication when at home. Serves as the primary nursing liaison for the development of a total care plan to patient and family. Supports the development of students, new staff and colleagues and supports shared governance through practice-based teams and structures. Organizes orientation, training, preceptor initiation, preceptor selection, preceptor training and evaluation period of new staff in department. Creates an environment of open dialogue, inquiry and continuous development by asking for feedback and improving practice. * Assumes responsibility for day-to-day operations of the practice while continuing to function clinically when necessary. Serves as the departmental role model in all aspects of patient and community care. Oversees program and patient coordination activities to ensure program goals and objectives are achieved by providing direction to staff, maintaining efficient operations and overseeing quality of patient care provided. Develop and optimize the clinical care tools of the practice and the physicians (scheduling templates, space, staffing mix and patient care protocols) that improve the quality of the overall product delivered to the patient. Determine and flex staffing standards based on patient care needs, patient flow, medical acuity, procedures, staff skills and other clinical needs. * Develops and maintains productive working relationships internally and externally by demonstrating accountability for actions, enthusiasm, motivation and commitment to patients and colleagues. Resolves and manages conflict in a smooth and timely manner. Demonstrates an understanding of cultural differences and holds peers accountable for healthy relationships. Maintains a conscious balance between work and personal life and permits the same focus for staff members. Models safe work hours, time management and healthy lifestyle. Communicates with peers and management any safety hazards identified in the workplace. * Serves as a highly engaged and full partner on the care team and responds willingly to care team member needs for assistance and partnership. Oversees daily operations of practice with regards to patients, staff, physicians and ancillary support/departments. Assist management team or assume formal leadership responsibilities, when necessary, in the following: budget and financial formulation, staff levels and coverage, human resource management, departmental development and education and safety/regulatory reporting and preparation. Qualifications: * Minimum 3 years Nursing Experience required. * BSN preferred. * Rating of Solid/Strong/Good or higher on most recent performance evaluation. UPMC approved national certification OR master's degree or higher in Nursing required. Master's degree or higher in a related field in lieu of Master's or higher in nursing acceptable if a BS in Nursing is held. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patients' requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures. Licensure, Certifications, and Clearances: Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. CPR Certification. UPMC Professional Contribution required CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire * Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) * Registered Nurse (RN) * Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
