Clinical Pharmacist - Home Infusion

City/State Charlottesville, VA Overview Work Shift First (Days) (United States of America) Sentara is looking for a full time Clinical Pharmacist to join the Home Infusion team in Charlottesville. Shift: Mon - Fri (8am - 5pm) w/ on call rotation The Home Infusion - Clinical Pharmacist works in a high volume infusion pharmacy within the homecare setting. The Home Infusion Clinical Pharmacist: * Assures the appropriateness of all physician medication orders. * Manages patient drug therapy in accordance with the medical staff approved clinical practice standards. * Maintains current knowledge of drug use through continuing education. * Serves as the primary resource for drug information within home infusion pharmacy. * Serves as a resource for community health education. * Precepts pharmacy students as assigned.
