Nurse Care Manager (Nurse - Family Partnership)


General Statement of Duties The Forsyth County Department of Public Health is seeking a full time Nurse Care Manager. This position is with Nurse-Family Partnership, an intensive, evidence based, home visitation program designed for first time, low-income mothers visited throughout their pregnancy and the first two years of their child's life. This program adheres to a "strengths based" model, utilizing therapeutic relationships, mutual goal setting and problem-solving techniques. Distinguishing Features This Nurse Care Manager position serves as the Care Manager to coordinate services and educational efforts for Medicaid-eligible or uninsured clients. Work is distinguished by the complexity of serving medium to high risk clients who are often dealing with multiple social, medical and economic issues. The position requires strong advocacy and teaching skills, organizational and time management skills as well as a commitment to personal and professional growth. Program goals include but are not limited to improved pregnancy outcomes, improved child health, development, and increased economic self-sufficiency. The expected caseload is 25 clients. A combination of county car and personal car with mileage reimbursement will be utilized. Training in the Nurse-Family Partnership model is provided locally and in Denver, Colorado. The typical work schedule is Monday - Friday 8:00 a.m. - 5:00 p.m. with some flexibility required to best serve the clients. Minimum Education and Experience Graduation from a four year college or university with a BSN that included a public health nursing rotation and two years of experience in Med/Surg, OB/GYN, maternal, NICU, or child health care setting to include planning and/or counseling of patient care; or MSN from an accredited school of professional nursing and one year of experience in Med/Surg, OB/GYN, maternal health, NICU, or child health care setting to include planning and/or counseling of patient care; or Graduation from an accredited school of professional nursing and four years of experience in Med/Surg/Ob/GYN, maternal health, NICU or child health care setting to include planning and/or counseling of patient care. Must be licensed to practice as a Registered Nurse in North Carolina by the N.C. Board of Nursing. Valid driver's license required; this position is part of a home visitation program. Essential Duties and Responsibilities Essential duties and responsibilities include but are not limited to the following: Monitoring and evaluating client progress toward mutually established goals; Providing coordination of client's care, services and educational efforts emphasizing community collaboration; Meeting with clients to formulate and execute a plan of care and self-management; Utilizing targeted reports to identify clients who may benefit from care management; Screening clients to assign priority risk and entering the data into an approved documentation system; Assessing whether a client will benefit from care management based on projected outcomes, utilization or cost and risk factors; Completing an assessment on all clients with a positive risk factor or priority indicator; Communicating on behalf of the client with PMH, Personal Care Physician, DSS, Health Department, hospital staff, community providers and others providing care to the client and; Reconciling and monitoring client compliance with prescribed medications(s).
