JOB SUMMARY: Under the direction of the Nurse Manager, the RN Case Manager communicates and collaborates closely with the physicians, outpatient and inpatient Nurse Managers, wound care staff and other members of the healthcare team to provide consistent and quality care for patients with wounds. Working with a high degree of autonomy, the RN accepts responsibility and accountability for assigned patients as a care manager to plan, implement, and evaluate the care for patients. The RN works within the framework of the State Nurse Practice Act and follows all partner facility, Wound Program, and nursing philosophies/standards in the performance of job duties and responsibilities. He/she complies with hospital and nursing philosophy and standards of the Wound Program performing job duties and responsibilities and will demonstrate competence in these key job areas through the ability to perform job functions, including, but not limited to, the common examples listed. QUALIFICATIONS: * Licensed to practice in the state of Oklahoma * Two (2) years of experience in acute care nursing * Experience in ambulatory care setting highly desirable * CPR Certified or completed within 30 days of employment * Able to complete a two (2) man lift, wheelchair to exam table * Able to communicate effectively, both verbally and in writing PHYSICAL REQUIREMENTS: * Able to communicate effectively, both verbally and in writing * Must have strength and stamina for prolonged walking, standing, and dealing with stressful situations * Demonstrates adequate vision or correctable with glasses to read and write * Able to hear, or correctable with hearing devices, and able to talk to converse with others in person and * over the phone * Able to hear, see, and feel to accurately assess, monitor, and administer care * Able to frequently lift at least 60 lbs., reach, pull, push, bend, stoop, and kneel * Possesses fine motor skills and hand/eye coordination to give care (i.e.: insert catheters, start IV's, administer medications, and handle instruments * Able to work in latex burdened environment
