RN Case Mgr - Norcross, GA - over $100K - $5K Bonus

RN CASE MANAGER - Brookhaven/Sandy Springs Areas - HOME HEALTH $5k RETENTION BONUS SALARY - $89k PLUS MONTHLY BONUS PLUS RETENTION BONUS POSITION SUMMARY Is responsible for coordinating the care delivered to a caseload of patients and ensures care delivered is within quality and utilization guidelines. Assesses, plans, implements, monitor's and evaluates actions taken as part of a comprehensive plan of care to ensure outcomes of care meet or exceed standards. MINIMUM JOB REQUIREMENTS Education & Qualifications: Education: Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree or higher Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing and valid CPR certification. Must possess a current valid Driver's license. Previous Experience: PRN Staff must have previous home health experience. Knowledge/Training: Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health care services. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process and collaborating with interdisciplinary team. Demonstrate knowledge of home health proficiency in quality of clinical records, productivity, customer satisfaction surveys and leadership role with staff. Must be able to function independently and effectively in a home based work environment. Requires proven interpersonal, supervisory and management skills with ability to communicate effectively. Demonstrates self-initiative, proven decision making skills and time management skills. Ability to operate computer equipment, FAX and copier machines as well as maintaining communication with Company telecommunication systems. Computer Skills: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs. Transportation: Must have valid transportation and current valid auto insurance coverage as required by VNHS policy. #VNHSJOB1
