Communicable Disease & Emergency Preparedness Program Coordinator -RN Required


Center for Human Development, Inc. is seeking a full-time Communicable Disease and Emergency Preparedness Program Coordinator to coordinate Communicable Disease programs to protect and improve the health of Union County residents through prevention, education, and the provision of direct home or office services in the areas of communicable disease investigations, testing, and emergency planning into other community services. This position fulfills the requirements of the Oregon Health Authority Public Health Emergency Preparedness (PHEP) Agreement. The PHEP Program shall address mitigation, preparedness, response, and recovery phases for public health emergencies through plan development and revision and response activities. The Requirements: * Must be at least 18 years of age * Licensure as a registered nurse in the state of Oregon. * Experience and training may include graduation from an accredited school of nursing, with a BSN, and supplemented by training and/or experience in public/community health nursing. * Excellent organizational skills * Work independently; self-directed and self-motivated * Interpersonal relations; builds networks relationships with participating counties, early learning hubs, EOCCO hospitals and clinics * Knowledge and experience of Evidence Based Practices and population-based interventions. * General knowledge of maternal and child health and other areas of Public Health. * Networking skills * Influence skills; work effectively within all levels of an organization * Strong leadership skills * Demonstrated community organizing, community development, and collaboration knowledge and skills. * Strong oral and written skills. * Must pass all criminal history check requirements as required by ORS 181.536-181.5337 and in accordance with OAR 407-007-0200-407-007-0370 and drug screening. * Must possess a valid Oregon Driver's license and be insurable for client service purposes, travel between business offices, and to attend required meeting and/or trainings. Essential Functions: Work with Public Health Program Lead on communicating and disseminating health risks information, including the following measures: * Encourage and provide means for reporting, monitoring, investigation and controlling communicable diseases and other health hazards through coordinated medical environmental and epidemiological interventions. * Community outreach and education. * Exercise the statutory responsibility to perform communicable disease control measures required by Oregon Statues and Oregon Administrative Rules. * Begin communicable disease investigations, including suspect cases, within 1 working day, or alternate time frame as defined by disease-specific protocols. Investigation and contract management, will be complete, documented and reported to Department of Human Services, Health Services. * Collaborate and cooperate with the state and other counties in outbreak investigations. * Conduct on-going surveillance and analysis of the incidence and prevalence of communicable diseases in consultation with Acute and Communicable Disease Prevention section of the Oregon Health Authority Public Health Division. Prevention section of the Department of Human Services, Health Services. * Develop and implement Public Health Emergency Preparedness (PHEP) work plan in accordance with Oregon Health Authority (OHA) requirements to provide public health capacity during outbreaks of disease and other public health emergencies. * Develop and maintain close working relationships with emergency response stakeholders/communities within Union County to better support and integrate public health, medical care (including mental health/behavioral health), and other community-based organizations, including tribes. Includes facilitation of local Union County Emergency Preparedness Coordination Group. * Maintain training records for public health preparedness and when training needs are identified facilitate access to required training for staff and partners. * Identify vulnerable populations and develop communication strategies to ensure these populations will be reached in the event of an emergency. * Attend all required meetings and training while maximizing distance learning and teleconferencing possibilities as it relates to public health preparedness. * Assure 24/7/365 emergency contact capability and serve as CHD's Health Alert Network Administrator. * Assure that all reporting requirements associated with the PHEP Program are met. * Must maintain punctual and regular work attendance. * Must have the ability to maintain positive and professional working relationships with co-workers, clients and other agencies. * Could require occasional weekend and after hour work.
