PeaceHealth is seeking a RN Annual Wellness Visit for a Full Time, 1.00 FTE, Day position. The general salary range for this job opening at PeaceHealth is $44.25 - $66.37. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc. Spotlight on PeaceHealth Sacred Heart Medical Center at Riverbend | Springfield, Oregon A Better Workplace Community The Annual Wellness Visit RN works in collaboration with the primary care provider and all members of the health care team. Primary responsibilities include review of identified Medicare patients appropriate for Annual Wellness Visits (AWV) and execution of guided processes to complete the AWV. Assesses healthcare needs that require additional follow up by primary care provider and assists patients in scheduling appropriate follow-up. Knowledgeable of community resources and assists patients in connecting with those resources. Serves as patient advocate and assists in identification and improvement of service delivery. This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations. Details of the position: * Partners with Payor Specialists to identify patients appropriate and due for Medicare AWV. * Documents patient care in the healthcare record according to established standards. * Develops and shares knowledge of AWV eligibility and documentation requirements. * Follows AWV protocols and standing orders including completion of preventive services that may result from AWV. * Develops knowledge of Medicare preventive services and clinical guideline recommendations for those services. * Addresses open quality gaps with patients and works collaboratively with patient towards closure of those gaps. * Acts as an advocate for patients and helps them to obtain their maximum level of health. Provides education to patients about their specific conditions and needs. * Consults with the clinical staff and ancillary department staff to eliminate barriers to the efficient delivery of care. Identifies service delivery problems and potential for effective patient management intervention. * Maintains professional communications and collaboration with all members of the interprofessional healthcare team and other stakeholders to improve coordination of care. * Performs other duties as assigned. What you bring: * Accredited School of Nursing required. * 1 year Preferred: experience as a registered nurse. * Required: Registered Nurse in state of practice. * Required Upon Hire: Basic Life Support. * Preferred Upon Hire: Advanced Cardiac Life Support. * Demonstrated ability to promote positive interpersonal relationships and patient/family centered work environment (Required) * Basic computer skills including MS Office applications (Required) * Effective prioritization skills (Required) * Ability to provide health education to patients, staff and community (Required) * Knowledge of Medicare, JCAHO & HCFA regulations pertaining to ambulatory care (Required) * Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs (Required) Working Conditions Lifting * Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH). * Fine motor skills to be able to grasp and control medical equipment and perform precise procedures. * Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts). * Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally. * Ability to move around area with frequent sitting. * Bending/ stooping/ squatting/ reaching/ kneeling frequently. * Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Environmental Conditions * Exposure to biohazard, body fluids and airborne particles. * Must be able to complete tasks in a noisy environment. Mental/Visual * Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted). * Ability to communicate and exchange accurate information.
