Responsible for case management intake assessments and performing comprehensive assessment of health care needs. Duties: * Fulfill nurse protocol training requirements in HIV, STD, Immunization, and other relevant areas to function as an expanded role PHN. * Provide direct nursing care including physical assessments, education, treatment, and develop and document a plan of care consistent with the clients' level of need. * Screen and refer clients to other community service providers as needed. Assist with referrals and follow-up treatment planning as indicated. * Provide sexual health promotion and prevention education, including counseling clients and family regarding the importance of medication adherence for viral suppression. * Participate in staff meetings and assist with developing and implementing quality improvement projects as needed. * Maintain accurate medical records, documenting is a must, assist with data entry and quality assurance monitoring.
