Requisition No: 841215 Agency: Department of Health Working Title: REGISTERED NURSE SPECIALIST - 64082002 Pay Plan: Career Service Position Number: 64082002 Salary: $62,000 annually Posting Closing Date: 12/06/2024 Total Compensation Estimator Tool Open Competitive Your Specific Responsibilities: This is a Registered Nurse (RN) Specialist position at the Florida Department of Health in Hendry/Glades County with the Healthy Start program. This position provides home visitation services including health promotion, health education, and referrals to at-risk pregnant women and infants. The incumbent is also responsible for assisting the Program Director with Quality Assurance (QA) and Continuous Quality Improvement (CQI) of the program(s). This position requires the incumbent to be self-motivated and possess the ability to work independently with minimal direct supervision. Position reports directly to the Program Director (Operations & Management Consultant II - SES). Certain immunizations and TB testing may be required. Incumbent will have access to records containing social security numbers in the performance of job duties. Healthy Start Case Management * Implements the Healthy Start program for families referred to and enrolled in Healthy Start in a manner consistent with the Healthy Start Standards and Guidelines and the Healthy Start Coalition contract. * Responsible for outreach, initial contacts, initial assessments, and ongoing care coordination services for families via letter, phone calls and home visits. * Provides services in a location that best meets the needs of the family through home visits, clinic visits, and visits in alternative settings. * Provides the Healthy Start services of care coordination, parenting support/education, childbirth support, smoking cessation, breastfeeding support, child growth and development, nutrition principles and the benefits of immunizations. * Promotes basic health principles and preventive care related to disease control, maternal/child/fetal health and family planning. * Provides education to families on child growth and development, nutrition principles and the benefits of immunizations. * Responsible for the development of Family Support Plans. * Client assignments may include issues related to substance abuse, domestic violence and teen pregnancy. * Provides appropriate referrals to client for community resources. * Assists families in assuring access to prenatal or infant care and community resources. * Documents service delivery as required. * Assigns clients a level in the Well Family System (WFS), prioritizing client caseload according to client risk status. * Quality Assurance/Quality Improvement * Assists with the coordination of Quality Assurance (QA) and Continuous Quality Improvement (CQI) programs. * Assists with evaluation of Family Support Workers (FSWs) including chart and documentation reviews as per the Healthy Start Standards and Guidelines. * Community Involvement * Acts as a liaison with community agencies and service organizations. * Collaborates with the Department of Children and Families and other community partners in the follow up of referred at-risk families. * Attends scheduled meetings, training sessions, and outreach events. * Represents Healthy Start in Hendry/Glades in the Healthy Families/Healthy Start Advisory Committee meeting. * Promotes public awareness of the Healthy Start Program and develops and maintains community relationships to support client referrals. * Performs other related work as assigned. Required Knowledge, Skills, and Abilities: Knowledge of nursing and public health principles, practices, and techniques related to maternal child health. Ability to participate in the planning and implementing of treatment plans for clients. Ability to work independently and to plan, organize, and coordinate work assignments. Ability to establish and maintain effective working relationships with others and to effectively interview and assess the health care of clients. The ability to advocate on behalf of clients, communicate verbally and in writing with health care providers and other community resource agencies, work with children and families in their home environment, open and check emails daily, use Microsoft Office computer programs and online data systems, actively listen and effectively communicate, work with culturally diverse families, maintain professional boundaries, accept individual differences, adapt to change, travel throughout Hendry and Glades Counties, and maintain a flexible schedule to meet the needs of clients served. Qualifications: Minimum: * Willing to work during or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent of this position includes working in Special Needs or Red Cross Shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural * Licensure as a Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 64B9. * Bachelor's degree OR three or more years of public health and/or maternal child health experience.
