PeaceHealth is seeking a RN - Hospice for a Per Diem/Relief, 0.00 FTE, Evening position. The general salary range for this job opening at PeaceHealth is $43.88 - $69.54. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc. This position is represented by a collective bargaining agreement. There may be more than one opening on this posting. Job Summary Responsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation. Essential Functions * Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services. * Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment. * Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes. * Develops and clinically manages the Home Health Aide's care plan according to the patient's evolving personal are needs. * Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum. * Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care. * Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines. * Assist with the plan and implementation of family Bereavement services for the following patient death. * Performs other duties as assigned. Qualifications Education * Accredited School of Nursing Required: Nursing and * Bachelor's Degree Preferred: Bachelor of Science in Nursing. Experience * 2 years Required: Acute clinical or home health/hospice nursing experience and * 1 year Required: Home health experience preferred or will orient to point of competency and * Required: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs Credentials * Required : Registered Nurse in state of practice and * Required : Drivers License and proof of auto insurance and * Required Upon Hire: Basic Life Support Skills * Basic computer skills including MS Office applications (Required) * Well organized, flexible and able to function independently (Required) * Cooperative working skills, leadership skills, and interpersonal skills and ability to work effectively with a wide variety of patients, families and staff (Required) Department / Location Specific Notes PHOR and SIUSLAW: * RN Home Health job classification in ONA bargaining unit. Home and Community: * Will accept valid driver's license in US or Canada and proof of auto insurance. Working Conditions Lifting * Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH). * Fine motor skills to be able to grasp and control medical equipment and perform precise procedures. * Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts). * Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally. * Ability to move around area with frequent sitting. * Bending/ stooping/ squatting/ reaching/ kneeling frequently. * Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Environmental Conditions * Exposure to biohazard, body fluids and airborne particles. * Must be able to complete tasks in a noisy environment. Mental/Visual * Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted). * Ability to communicate and exchange accurate information.
