Overview UF Health Leesburg Hospital has been serving the health care needs of our community for 58 years. UF Health Leesburg Hospital also offers advanced orthopedic surgery, stroke care, emergency services as well as labor and delivery care. The hospital is part of UF Health Central Florida, a not-for-profit health care system and the largest most comprehensive provider of health care services in the region. UF Health Central Florida provides inpatient acute hospital services at UF Health Leesburg Hospital and UF Health Spanish Plaines Hospital, inpatient rehabilitation services at UF Health Rehab Hospital - The Villages and diagnostic laboratory services at several locations. Responsibilities The Case Manager utilizes professional nursing skills to assess patient and family scenarios for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care. Assessment, planning, intervention and reassessment are performed in a collaborative environment. The key goals of the Case Manager include proactive, individualized planning for patients progress that optimizes quality of care, utilization, cost and patient satisfaction. Qualifications Education: - Bachelors Degree Preferred - Diploma/Associates Degree in Nursing - Professional Certification in Case Management, preferred Licensure/Certification/Registration: - Florida Registered Nurse License Special Skills/Qualifications/Additional Training/Experience Required: Effective communication skills to facilitate contact with physicians, patients/clients, families and staff. Minimum of 5 years experience, Case Management experience preferred. - CCM certification preferred. Must be able to read, write, speak and understand English
