RN, Ambulatory Oncology Nurse Navigator
Department Overview The Oncology Nurse Navigator will work collaboratively with the Community Hem/Onc teams when a patient is referred to the department and throughout the cancer continuum. An oncology nurse navigator (ONN) is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The ONN will work with the ONS core competencies as the guide. The ONN competencies to be performed: (a) coordination of care, (b) communication to patient, families, and members of the healthcare team, (c) patient and family education (d) professional role. Function/Duties of Position * Works with the Providers to ensure that all baseline clinical information, both diagnostic and therapeutic, is gathered together in a systematic fashion prior to the patients visit. Works to ensure that new patients are prepared for the testing, evaluation and potential outcome of the visits ahead of time. * Collaborates with the Team Coordinators and Managed Care Coordinators to ensure that all appropriate business related information is gathered prior to the visit, authorizations have been obtained and pre-registration has taken place. * May be called upon to assign tasks to team members after assessing patient needs, staff skills and resources. * Collaborates with other members of the team to ensure that patient flow is expedited during clinical practice sessions. * Ensures that Provider orders that result from the new patient visit are recorded and executed. Works to ensure that laboratory and other diagnostic findings are available to clinicians in a timely fashion after tests are performed. Tracks the post visit flow information to ensure that plans of care are executed in a timely fashion. * Ensures that when hospitalization is required; those patients and their family members understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period. The RN Navigator is expected to troubleshoot any problems that patients and their families may encounter in preparing for or during their hospital stay. * Required Qualifications * Two years of experience in an adult oncology setting. * Recent adult oncology triage experience. * Competence in EPIC and beacon plans. * Associate or Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA).* BSN preferred.* Associate Degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire. * Current unencumbered Oregon RN license. * Current BLS certification issued by the American Heart Association * Professional demeanor. * Strong triage skills. * Organization skills. * Leadership ability demonstrated by role modeling. * Communication skills. Preferred Qualifications * Recent experience in an ambulatory oncology setting. * Experience in Oncology Nurse Navigation or Coordination. * At least one year experience providing nursing skills over the phone. * Demonstrated experience in patient and family education, including development of teaching materials. * Recent experience in an Ambulatory care setting. * Bilingual. * OCN certification. * Chemotherapy and Biotherapy Provider Certifications.
