Stroke Responder Nurse, Full-Time/Nights


Position Summary Stroke Responder Nurse is a specialized Registered Nurse who facilitates stroke alert process to all OH facilities excluding OHBFSP and associated FSED. They are part of an expert clinical team whose main goal is to facilitate the stroke alert process for rapid evaluation and decision making for treatment of acute stroke. Performs NIHSS patient assessment, obtains patient history and current medication history and reviews exclusion/inclusion criteria. Qualifications Education/Training • Graduate from an approved school of nursing • Advanced clinical knowledge in neurovascular with advanced clinical assessment skills Licensure/Certification • Maintains current State of Florida RN License. • Maintains current BLS/Healthcare Provider certification. • Neuroscience or Stroke certification preferred or must obtain within 1 year. • NIH Stroke Scale Certification • ASLS Certification required within 1 year of hire. Experience • A minimum of 1-year clinical RN experience with care of neurovascular patients Responsibilities • Documents patient care in a knowledgeable, skillful, and consistent manner, meeting all required and regulatory standards in electronic clinical health record (CHR). • Responds to all OH stroke alerts (inpatient/ED/telemedicine) in an expedited manner. • Performs NIHSS assessment and collects all data required for treatment decision. • Communicates and collaborates with medical staff and interdisciplinary team to effectively plan and manage the stroke care in the acute phase for patients presenting with symptoms consistent with stroke. • Reports clinical assessment, medical history, current medications, inclusion/exclusion criteria, pre stroke-mRS, and all other relevant information to Vascular Neurologist and any other provider involved in patient care. • Collects stroke data and perform real time chart review to ensure stroke core measures are met. • Educates nurses regarding stroke nursing care, core measures, and assessments & monitoring. • Provides patient with stroke education to include signs & symptoms of stroke, individualized risk factors and treatment. • Respects diversity by building respectful relationships with all team members and customers. • Functions as a patient and family advocate. • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit/department. • Serves as a role model for staff and supports the hospital and department's goals and strategies. • Demonstrates knowledge of the principles of growth and development over the life span, assesses the data reflective of the patient's status, and interprets the necessary information needed to identify each patient's requirements relative to his or her age specific needs. • Coordinates the care and delegates as appropriate to other team members on a defined group of patients. • Demonstrates competency in nursing skills and use of patient care/unit equipment as defined by unit/department-specific requisite skills. • Prioritizes patient care in an ongoing manner in accordance with Evidence-Based Practice Standards of Care and Stroke Protocols. • Practices effective problem identification and resolution. • Communicates appropriate information regarding patient condition or unit concerns to other health care team members. • Demonstrates caring practices by providing a compassionate and therapeutic environment for patients and their families. • Demonstrates awareness of legal issues and patients' rights. • Collaborates with the education department and nursing leadership team to effectively transition and support new team members and/or students. • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state, and local standards. • Maintain compliance with all Orlando Health policies and procedures. Other Related Responsibilities • Proficient in building interpersonal relationships using excellent communication skills and team building strategies. • Practices effective problem identification and resolution utilizing evidence-based practices. • Partners with the nursing leadership team and Medical Director of Stroke to identify professional development needs. • Complies with all regulatory and accrediting requirements.
