Title: Triage Nurse Location: Totem - 1438 SE Division, Portland, OR 97202 Schedule: This is an in person position, Monday through Friday 8:30am-5pm. Position Overview: This position provides nursing assistance within a mental health outpatient setting. The position requires the ability to meet with clients in the community and assist clients with medical appointments. The position serves individuals who have been diagnosed with severe mental illness who are receiving mental health services through NARA NW. What you will do: * Perform nursing and clinical assessments with each client. * Provide care coordination, to include effective communication, for the client within the treatment team, within the agency, and with outside community partners. * Manage and assist with client medical and psychiatric medications, including injections. * Maintain timely, accurate, and chronological documentation in client electronic health records. Qualifications We would like to hear from people that have: * Licensed as a Registered Nurse with the State of Oregon, BSN Required. * Registered QMHP-C, or the ability to become a QMHP-C through MHACBO within 6 months of hire. * Experience providing direct services to individuals living with severe and persistent mental illness, preferably in a behavioral health setting; community-based services a plus. * Directors Designee in Multnomah County preferred or ability to become one after hire. * Must have and maintain a reliable vehicle. This also includes maintaining a valid driver's license, valid automobile insurance, and valid state vehicle registration.
