Discharge and Continuity of Care: * See patients in the hospital discharge care continuity clinic to ensure a smooth transition post-discharge. * Collaborate with the care team to manage follow-up care plans and address any issues arising post-hospitalization. * Monitor patient outcomes and adjust care plans as necessary to optimize patient recovery. Documentation and Compliance: * Maintain accurate and timely documentation of patient encounters, assessments, and care plans. * Ensure compliance with hospital policies and procedures as well as state and federal regulations governing APRN practice. * Participate in quality improvement initiatives aimed at enhancing patient care and reducing unnecessary delays in the ER, hospitalization and length of stay, as well as hospital discharge process. Education and Training: * Provide education to patients and families regarding epilepsy management, medications, and follow-up care. * Mentor and educate nursing staff, residents, and other healthcare professionals about inpatient neurology and epilepsy management. * Participate in continuing education to remain current with best practices in neurology and epilepsy care.
