GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES: The Registered Nurse works in conjunction with a medical provider, to provide professional, team based medical care and nursing support in a clinical setting. Under direct supervision of the Provider , the nursing staff plays a critical role in the delivery of quality, patient- centered medical care within the context of a federally (HRSA) qualified community health center (FQHC). SPECIFIC DUTIES AND RESPONSIBILITIES: A. 1. Communicates with patients in a courteous and professional manner in face to face encounters and via the telephone. 2. Educates the patient and family member on the patient's diagnoses' and health care self-management, within scope of knowledge and authority. B. 1. Participates with the Provider in the implementation of the plan of care and multidisciplinary patient education. C. Promotes the Team-based Care Model by participating in daily huddles and Patient Centered Medical Home (PCMH) bi-weekly team meetings to communicate and collaborate on patient issues and needs. D. 1. Conducts patient screening for hearing, sight, vital signs, blood pressure and other routine procedures. 2. Assist the Providers with in-office test and procedures as ordered. 3. Administers medications including IM and Sub Q injections as ordered by the Provider. 4. Initiate and monitor the infusion of IV fluids if needed. 5. Documents clinical activities and patient interactions in the Medical Record. 6. Collects specimens from patients, performs in-house laboratory testing, and processes specimens and requisitions for transport to the outside testing facility. 7. Stocks exam rooms and special treatment rooms with needed supplies and indicates necessity for ordering supplies on the appropriate supply order form. 8. Demonstrates enthusiasm and commitment toward continuous learning and developmental opportunities. 9. Returns patient phone calls promptly. 10. Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary. 11. Functions as a triage nurse. 12. Interacts with other agencies/medical entities in regards to patient medical information and documenting referrals as necessary. 13. Participates in education of nursing students as assigned. 14. Physical attendance is an essential element of the job and necessary to perform the essential functions of the RN position. 15. Other clinical duties and responsibilities as assigned by the lead clinical person. E. 1. Attends and participates in staff and other JHC meetings as required. 2. Supervises other nursing and support personnel as assigned. F. 1. Attends all nursing staff meetings and participates in the JHC QI program. G. 1. Attends all mandatory OSHA infection control trainings and follows general OSHA guidelines for infection control, including "Universal Precautions". 2. Abides by all JHC infection control policies, including acute response measures in the setting of a public health concern or disease outbreak. H. 1. Performs Annual Skills Evaluation and signs the "Nursing Competency Skills Checklist". The Registered Nurse reports to, is supervised by, and is evaluated by the Director of Nursing with input from clinical personnel. The Registered Nurse may be asked to supervise a Licensed Practical Nurse or Medical Assistant. The evaluation of work performance will be on-going, and will be carried out by a peer Registered Nurse or Provider. It will include the specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will normally be performed annually, but may be initiated at other times by the Registered Nurse, peer Registered Nurse or Provider. 1. Must be licensed with the Virginia State Board of Nursing with a current RN license. 2. Must have graduated from an accredited nursing program. 3. Should there be a work experience qualification? 4. Ability to work a flexible schedule. This position is a FLSA Non-Exempt position. This is an OSHA high risk position. This is an ADA negotiable position.
