If so, we have an exciting opportunity for you to join a talented and passionate team!  We are seeking a Community Health Nurse, pediatric experience is a plus, to provide a variety of professional population-focused community and public health nursing services in health promotion, prevention and disease control programs; and to perform related duties as assigned.  Brief Description of duties:  * Performs independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach.  * Provides patient education as an integral part of the services offered.  * Acts as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention.  * Communicates with providers regarding determinations and situations, documents events appropriately.  * Prepares and administers oral, subcutaneous, intra-muscular and intravenous medications.  * Independently instructs and demonstrates to patients how to provide self-care. * Instructs patient in self-medication of prescribed pharmaceutical agents.  About the Division At Lane County Health & Human Services, we believe healthy people are the foundation of healthy communities. Community Health Nurse 1: Training & Experience: * Associates degree in Nursing from an accredited college or university is required. College-level coursework in the social sciences, and additional nursing training within a community or public health curriculum desirable.  * Nursing experience in public health or community health care services desirable.  Community Health Nurse 2: Training & Experience: * Bachelor of Science in Nursing degree from an accredited college or university. Any equivalent combination of experience and training that provides the required knowledge and abilities is qualifying.
