City/State Norfolk, VA Overview Work Shift First (Days) (United States of America) Sentara is looking for a Registered Nurse (RN) to join the Sentara Palliative Care Specialists team. The RN Palliative Care functions as a clinical leader under the direction of SMG Care Management Services and is responsible to designated practice manager(s) and practice physician lead(s) to provide targeted population health management for SMG patients with or at-risk of developing chronic disease(s) and/or condition(s) Responsible and accountable for the management and coordination of patient care and services to meet identified needs to assure quality patient outcomes. Coordinates and collaborates with providers and other clinical staff to provide comprehensive coordination of care, and provides delegation and supervision of tasks to LPNs and MAs. Job Requirements: * RN-Associate's Degree OR RN-Bachelor's Level Degree OR RN-Diploma (Non-degree) * Required: Basic Cardiac Life Support, Registered Nurse * Required: 3 years nursing experience required. * Required: Specialty certification required within one-year of eligibility (ACM, CCM, CMCC or RN-BC). New Hire Education Requirement: All Registered Nurses who do not have their BSN will be required to sign a BSN Agreement committing to successfully obtain their BSN within 5 years of hire. 3 years current nursing experience; ambulatory experience preferred. Specialty certification required within one-year of eligibility (ACM, CCM, CMCC or RN-BC). BLS required within 90 days of hire.
