MFM Complex Care Manager
JOB SUMMARY: The MFM Complex RN Care Manager works collaboratively as an active member of the interprofessional team to provide patient centered care. The MFM Complex RN Care Manager will systematically and continuously collect and assess data related to patient health status to develop, execute, and evaluate the plan of care. They address all patients, visitors, co-workers, and care-providers with courtesy and respect. They assess patient records for MFM referrals to aid in appropriate scheduling for consultation or follow up; work with referral management to ensure necessary records are available. The MFM Complex RN Care Manager will collaborate with other entities or providers to coordinate patient services, work collaboratively with Mission Hospital's Perinatal Nurse Navigator and link patients with community-based services. They provide patient education as it relates to specific MFM needs. The MFM Complex RN Care Manager is an active participant in the Perinatal/Neonatal monthly meeting; assist in updates on the Perinatal Tracker List. They support the MFM Specialty and High-Risk OB Clinic and provides patient care in the following areas: telephone triage, patient education, maintenance of the electronic medical record, scheduling, medication administration, and direct patient care. Ideal candidates will have passion for and experience with OB/GYN clinical services and/or care management. SPECIFIC RESPONSIBILITIES: * Serves as an expert in high-risk maternity care for the care management team. Ensures team members are trained in programmatic requirements and care management services according to patient needs and risk. * Executes nursing interventions by collaborating with the interprofessional team incorporating appropriate standards of care and practice and patient outcomes. * Assumes responsibility and accountability for effectively managing nursing care of individual patients. * Identifies learning needs of the patient and educates by adapting to patient learning style utilizing teach back method. * Provides care that is safe, ethical, patient-centered culturally congruent and evidence-based. * Identifies and responds to operational or clinical performance opportunities and records/documents according to established practice guidelines. Provides feedback and escalates cases as appropriate to department leadership or physician advisors. * Assists with processes and standardization across departments to meet clinical quality metrics. * Positively impacts patient satisfaction by eliciting and incorporating the expectations of patients, physicians, caregivers, payers and vendors into performance improvement initiatives. * Screens, assesses, coordinates, facilitates, and communicates a focused patient centered plan of care in conjunction with the patient, family and caregivers that includes safe and efficient transitions of care when applicable for assigned patient population. * Clinical Effectiveness: Provides for clinical and therapeutic interventions and manages variance to the plan. Collaborates with the clinical team and medical providers to initiate the anticipated peripartum and discharge plan for the patient and ensure progression of the plan. * Triage, telephone and in-person. * Review and act on abnormal test results. * Initiate and monitor intravenous (IV) infusions. * Assist or perform procedures (e.g. amniocentesis, chorionic villus sampling, NSTs, dressing changes, subcutaneous or intra-muscular injection, etc.) * Assist with medication management. * Complete accurate and timely documentation in the electronic health record. * Assist with management of urgent and emergent patient care situations. * Serve as a resource and mentor for others. * Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness. * Telecommuting as needed for operational needs. * Other duties as assigned. SPECIFIED SKILLS * COMPUTER * Excellent skills in Microsoft Office including Word, Excel, PowerPoint, and database applications required. * FOREIGN LANGUAGE * Spanish speaking skills preferred. * Excellent verbal and written skills. * Staff must demonstrate appropriate judgment in handling daily assignments. * Must demonstrate appropriate customer service orientation in all interactions within the organization and with patients, co-workers, care providers, and visitors. PHYSICAL DEMANDS * Frequently stands, walks, uses hands to handle or feel, reaches with hands and arms, bends, talks, and listens. * Sometimes sits, climbs, and stoops or kneels. Lifts up to 50 pounds maximum with frequent lifting/and or carrying objects up to 25 pounds. SUPERVISORY RESPONSIBILITIES: N/A EDUCATION AND EXPERIENCE * MINIMUM QUALIFICATIONS: Associate Degree in Nursing or equivalent required. Graduation from a certified RN program required. * PREFERRED QUALIFICATIONS: Two (2) years relevant experience or comparable combined experience and/or education preferred. 1-2 years of nursing experience in an OB/GYN setting preferred. REQUIRED LICENSES: * Current North Carolina nursing license or Board eligibility required. * Current CPR certification required. SCHEDULE: Regular attendance on-site is an essential function of this position. Typical business hours are Monday - Friday, 8:00 am to 5:00 pm (or flexed to best meet the needs of the clients and/or the Division); 40 hours per workweek; weekend, holiday, or evening coverage is occasionally required. Work hours will need to be flexible in order to respond to special work assignments, or evening activities, as requested by the team leader.
