Patient Navigator, RN - Oncology
Overview Halifax Health is seeking a Patient Navigator for our Oncology Services Department Summary The RN Patient Navigator is responsible for coordinating the care and services, advocate for the patient, and facilitates timely access to quality health and psychosocial care through the cancer care continuum. Job Qualifications * RN - State of Florida, or Degree from an accredited school of nursing, or Certified Navigator through National organization, or bachelor degree preferred. * CEU's related to practice areas recommended. * Leadership development courses preferred. * Minimum of 2-5 years experience in Oncology services. Job Duties and Responsibilities * Completion of general and nursing orientation. * Professionalism in interpersonal communication skills with patients, families, and colleagues * Completion of annual computer based learning modules [CBL's] * Initiate patient contact after referral for services * Meet with patient and family to discuss individual needs, services, and follow up appointments as needed. * Assist with coordination of services throughout the navigator involvement which include: * appointment planning * referral to scheduling department, financial services, ancillary services, or other professional services * Document all information, meetings in computer based system. * Provide handoff communication between departments, physicians, and team members. * Make appropriate referrals to community and other resources. * Attends and participates in appropriate weekly conferences or cancer committee. * Participates in ongoing programs as directed by leadership. * Works with patients to ensure ability to obtain their medications. Involves contacting insurance companies, drug companies, and copay foundations to assist with obtaining self-administered medications. * Demonstrates professional development, accountability, and leadership in the performance of their professional nursing practice. * Adheres to our Cornerstone standards and hospital mission and values at all times. * Reports to work when scheduled (Refer to Attendance policy). * Maintains a professional appearance that demonstrates a positive image (Refer to Professional Appearance Expectations policy). * Maintains patient confidentiality and acts in an ethical manner at all times. * Follows legal and ethical guidelines in providing nursing care.
