Registered Nurse RN Case Manager for Rehab Unit

JOB SUMMARY: A professional position where the case manager collaborates with interdisciplinary team members to provide discharge planning services to patients and families. This position is a part of the Case Management Department, and the primary assignment is the Rehab Unit. QUALIFICATIONS: * Master's Degree in Social Work or related field of study from an accredited college program OR Graduate from an accredited school of nursing (BSN degree preferred). * Minimum 2 years of clinical experience preferred. * Case management experience preferred. * Current license to practice in the state of Oklahoma. * Strong skills in oral and written communication. PHYSICAL REQUIREMENTS: * Able to speak clearly and distinctly with staff, physicians, patients, and families. * Able to extensively utilize telephone for communication. * Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule. * Able to sit/stand/walk for extended periods of time throughout the work schedule, with some light lifting and bending. * Demonstrates adequate vision, or correctable with glasses/contacts, for reading and preparing written documents. * Able to hear, or adequate hearing with corrections, spoken communications from staff, physicians, patients, and families as well as pages. * Demonstrates adequate manual dexterity to prepare written documents. * Able to work in a latex burdened environment.
