Case Manager and DME Coordinator

Responsible for the case management including assessment/reassessment, service planning, and monitoring of Morton patients, in addition to DME referrals. Essential Duties and Responsibilities: * Responsible for processing and coordinating DME referrals. * Identifies, assesses, and works with high-risk patients. * Conducts initial screenings to determine eligibility. * Develops comprehensive treatment plans. * Communicates with care team. * Engages with patients to assure understanding of goals and assists in meeting goal and monitoring progress. * Coordinates delivery of service. * Leads Transition of Care (TOC) activities. * Refers all identified social drivers of health to Community Health Worker or designee. * Coordinates with interdisciplinary team and documents all services provided. * Other duties as assigned Job Qualifications: * Current OK nursing (LPN or RN) license in good standing is required. * 2+ years of demonstrated clinical experience in healthcare. * 1+ years of demonstrated Clinical Case Management or Care Management experience is required. * Current BLS/CPR certification required. * Demonstrated ability to work effectively with staff and public. * Exceptional benefits including health, dental, vision, life, AD & D and long term disability insurance. 10 Paid holidays and 1 floating holiday in addition to generous PTO. 403 B Retirement plan with 4% match.
