Case Manager and DME Coordinator

Responsible for the case management including assessment/reassessment, service planning, and monitoring of Morton patients, in addition to DME referrals. Job Qualifications: * Current OK nursing (LPN or RN) license in good standing is required. * 2+ years of demonstrated clinical experience in healthcare. * 1+ years of demonstrated Clinical Case Management or Care Management experience is required. * Current BLS/CPR certification required. * Demonstrated ability to work effectively with staff and public.
