Prevention Care Team RN
SUMMARY: This position involves 0.5 FTE of Prevention Nurse responsibilities and 0.5 FTE as a Care Team RN (5 half days per week in each role). As a Prevention Nurse, this individual helps implement preventive health activities at Esperanza Health Center. The Prevention Nurse provides education, support, and clinical follow-up for patients who are prescribed PrEP (pre-exposure prophylaxis for HIV). The Prevention Nurse works with the Clinical Medical Director to support chronic care initiatives, including diabetes care management and education, mental health screening initiatives, and other targeted screening activities as directed. Together with primary care practitioners and outreach coordinators, the Prevention Nurse will develop and implement care plans and patient education, and work with team members to provide linkages for the various health and social needs of patients. As a Care Team RN, this individual provides direct patient care in a care team through participating with the clinician in visits (especially acute co-visits, new patient exams and hospital follow-up visits), performing RN procedures in the care team, addressing needs of walk-in and call-in patients for a care team, providing patient education, integrating spiritual care, and coordinating care for complex patients on the care team. DUTIES AND RESPONSIBILITIES AS PREVENTION NURSE: Preventative Services: * Collaborate with the Prevention Nurse Care Manager and Prevention Specialists for HIV prevention within the health center. * Participate in QA support, SBIRT, and other prevention programs. * Supports tertiary prevention efforts among patients with diabetes, including but not limited to the following: * Collaborates with PCP to assist with diabetes education, insulin titration, diabetic foot care, and other care needs. * Maintains up-to-date information of diabetic supplies and medications covered by various insurances. * In collaboration with prevention specialists and PCPs, assists with ordering supplies, including submitting prior authorizations as needed. * Assists with referrals for patients with diabetes to relevant community resources, such as nutrition classes. * Perform follow-ups with patients prescribed PrEP and provide medication education, follow up on side effects, and monitor standing orders for PrEP labs * Handle STI phone calls * Assist in providing PrEP updates and continual training for clinicians and Care Team RNs with an assigned Associate Medical Director championing PrEP workflows Other: * Ability to integrate faith and clinical practice. * No on-call responsibilities; no weekend hour required * Other duties and responsibilities as assigned. DUTIES AND RESPONSIBILITIES AS A CARE TEAM RN: * ASSISTS CLINICIAN IN DIRECT PATIENT CARE * ADDRESSES NEEDS OF WALK-IN AND CALL-IN PATIENTS FOR CARE TEAM * COORDINATES MULTI-SPECIALTY CARE AND FOLLOW-UP CARE FOR CARE TEAM * PARTICIPATES IN PATIENT HEALTH EDUCATION * Provides patient education on topics such as: diet and nutrition; general patient education; self-administration of injectable medications; prevention and treatment of asthma; use of glucometer, etc. * SPIRITUAL CARE * Provides effective spiritual intervention with patients, sharing hope and salvation through Jesus Christ, sharing helpful Biblical instruction and scriptures, praying for pts, and connecting/referring to Christian churches as appropriate and according to the leading of the Holy Spirit. EDUCATION, KNOWLEDGE, SKILLS, AND QUALIFICATIONS: * Registered Nurse license required; BSN preferred * Good oral and written communication skills - Bilingual English/Spanish required or willingness to learn * At least 1-2 years of experience providing clinical services * Supervisory experience preferred * Maintains CPR certification. * Solid interpersonal skills, including ability to deal with people politely, even when they are impolite. Must communicate warmth and be able to put people at ease. * Ability to remain calm under pressure. * Ability to take initiative in communication, being clear and direct, in order to effectively communicate with other staff. * Some computer experience - word processing and/or other. * Demonstrated ability to handle many details and multiple priorities. Must show ability to organize, process, and verify details. * Must have a solid record of handling confidential information, as well as demonstrating trustworthiness and honesty in all transactions. * Solid character references, as well as no criminal record. Also, a solid record of timeliness and reliability in a work setting. * Experience in community/outpatient setting and case management preferred. * Experience in serving the poor, urban environments, familiarity with North Philadelphia is preferred. Spanish immersion training is available should you qualify for this position but are not fluent in Spanish. EHC will provide further information.
