Our Expert Care. The job you have been looking for! AuthoraCare Collective is currently seeking a full-time RN - Kids Path for our Greensboro service areas. Job Summary: Manage RN case load for all/multiple services of Kids Path program (may include hospice, home health, CAP-C, perinatal services), in collaboration with other members of the interdisciplinary Team (IDT). Assess and identify patient/caregiver complications, establish patient/caregiver care goals and identify and implement interventions. Work with physician to establish and implement medical plan of care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching in collaboration with the assigned social worker and/or chaplain . Work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. Availability after-hours occasionally required at the discretion of the Kids Path leadership team. Program support responsibilities required in combination with field responsibilities. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Required Qualifications: * Graduate of an accredited School of Nursing. * Must be currently licensed as an RN in North Carolina and in good standings with NCBON * At least one year of experience required (two years preferred) in an acute or home setting. * At least one year of pediatric experience required. * Bachelor's degree in nursing preferred. * Familiarity with Medicaid, CAP, Case Management and Home health a plus.

$30.29/hour
