Admission RN - Hospice

Job Summary:# This individual would be trained and proficient in completing hospice consults and admissions for hospice appropriate patients in the Gaston Hospice service area.# This person would also provide hospice nursing services in the home/facility environment when needed.# The role also includes education and collegial relationship building with MD#s, facilities, nursing staff and the community.# The admission RN cooperates with other disciplines to give high quality total care to the patient in their home or in a contracted facility.### Qualifications:# Graduate from an accredited school of nursing and current RN license to practice in NC (NC license or multi-state (compact) license). A minimum of 1 year clinical experience in an acute care setting with oncology background preferred.## Clinical experience must have been within the last 3 years unless employed currently in home care setting.## Utilizes good clinical judgment and skill in the nursing management of clients with chronic or acute illness.# Demonstrates the ability to function independently and to provide guidance and supervision of C.N.A#s.# Has good time management skills, great attitude, flexibility, is self-motivated and conscientious. Must maintain BCLS certification.## Valid driver#s license, car insurance and reliable transportation to facilitate visits to home.# Intermediate computer skills required.## EOE AA M/F/Vet/Disability
