Stroke Program Coordinator RN
Programmatic initiatives for the stroke sub-service line and assure compliance with DNV stroke certification requirements. Serves as the point of contact for stroke treatment throughout the hospital, and works collaboratively with the Stroke Committee and other key stakeholders on all stroke efforts. Functions as a clinician, an educator, a consultant and a researcher with the primary responsibility for promoting quality care for stroke patients. Coordinate care of stroke patients in order to meet multiple service needs, ensure continuity of care, and eliminate fragmentation of care and services in all settings across the continuum. MINIMUM EDUCATION REQUIRED: Graduate from an accredited school of nursing. MINIMUM EXPERIENCE REQUIRED: Three (3) years of recent and relevant clinical experience related to stroke care required. Experience with data analysis and research required. MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License. NIHSS certification required. ADDITIONAL QUALIFICATIONS: Bachelor's or Master's degree preferred. Advanced certification in Neuroscience nursing (CNRN) or Stroke Certified Registered Nurse (SCRN) preferred. KEY RESPONSIBILITIES: 1. Coordinates accreditation efforts as directed. a. Coordinates task groups to work on identified areas of opportunity. b. Assures compliance with disease-specific certification requirements. c. Serves as the point of contact for stroke at the local entity as well as the American Heart Association, American Stroke Association, and National Stroke Association. d. Actively communicates and updates hospital Survey Accreditation Manager of changes and developments of stroke program. 2. Supports the administrative efforts of the overall program development as well as continued marketing development. a. Supports administrative decisions from hospital executive team. b. Coordinates with PR and Marketing any scientific and patient interest media relations. 3. Develops stroke program specific goals and assists in collection of data. a. Establishes performance improvement goals in conjunction with Directors or department managers as indicated. b. Works with department managers' staff to maintain and achieve regulatory and accreditation compliance. c. Assist individual departments with development and implementation of data driven performance improvement action plans. 4. Monitors and analyzes data, identifies trends and risk related issues and reports recommendations. a. Reviews, analyzes and interprets data from various sources to determine opportunities for improvement and identifies causative factors. b. Reviews data sources to identify outcomes, physicians practice patterns, or other potential problems that reveal variation. c. Communicates significant findings to stroke committee, key clinical areas and neuroscience service line. 5. Promotes the principles of performance improvement and uses the process of PDCA for stroke program. a. Explores strategies to promote cost effective resource utilization within the stroke population following established standards of care. b. Collaborates with the stroke team, key physicians and other health care disciplines in developing, implementing and monitoring value indicators. KNOWLEDGE, SKILLS, ABILITIES * Ability to work with physicians, administrative and clinical staff to achieve stroke program goals and objectives. * Excellent communication, presentation and critical thinking skills. * Experience in clinical and community education with an emphasis on public speaking. * Must be a self-starter who displays a high degree of accountability and follow through. * Exceptional assessment skills and ability to develop an individualized teaching plan for each referral. * Working knowledge of community resources.
