Student Health Services Nurse Case Manager

Rogue Community Health is looking for a Student Health Services Nurse Case Manager. This position is based at Eagle Point School District. This position may be expected to occasionally travel to all schools within Eagle Point School District as needed. This position is responsible to identify and assess health disorders among students and provide instruction in the maintenance of good health and disease prevention. Ensure vision and hearing screenings for students. Prepare classroom health plans in conjunction with student's medical provider and family and refer students to appropriate resources as needed. Work co-operatively with staff and parents to help remove barriers to good health and health care access with emphasis on the at-risk and special needs students. Our ideal candidate is committed to serving our patient population to facilitate patient empowerment and self-care. EDUCATION: Graduate from an accredited school of nursing; BSN preferred. Prior school nurse experience also preferred. EXPERIENCE: Minimum five years' professional nursing experience with three or more years in pediatrics and/or public health nursing preferred. Experience with electronic health records required. Licensure and Certifications: Current unencumbered Oregon license to practice as a Registered Nurse and Current Basic Life Support Card Spanish speaking and previous School nursing experience is desired. This position works full-time hours during the school year only with two weeks off at Christmas and the month of July off as well.
