This position coordinates the Minimum Data Set (MDS) system according to CMS certification and licensure standards on all residents.  Coordinates and serves as Team Leader for the Treatment Team on the assigned unit.   Serves as teaching role as needed to maintain skills for clinical disciplines on MDS and care planning.   This position is considered mandatory, which means employees are required to report to work during adverse weather and emergency conditions.   Longleaf Neuro Medical Treatment Center (LNMTC) is a certified Medicare and Medicaid facility nursing home that provides a continuum of care to both adults with severe and persistent mental illness that also have long-term medical conditions requiring residential, medical and nursing care, and to adults with a diagnosis of Alzheimer's or other related dementia.  LNMTC facilitates high quality skilled and intermediate nursing care throughout four Nursing Unit Floors and an Alzheimer's Unit that operates 24 hours seven days a week, with 1st, 2nd and 3rd shifts.        See more of Longleaf and what our employees have to say about it!    https://vimeo.com/239654418       ABOUT NC DEPARTMENT OF HEALTH & HUMAN SERVICES (DHHS):  The North Carolina Department of Health and Human Services (DHHS) serves the needs of the most vulnerable of North Carolinians and, in order to accomplish this, we hire only the most dedicated and caring individuals.
