Turn your passion into purpose. We are seeking a dedicated and compassionate Field-Based Licensed Practical Nurse (LPN) to join our dynamic team. In this role, you will provide essential nursing care in a variety of settings, including patients' homes and community-based environments. Your focus will be on delivering high-quality care, monitoring patient progress, and collaborating with interdisciplinary teams to support mental health treatment plans. WHAT IS YOUR SUPER POWER? * Patient Care: Deliver nursing care in the field, including medication administration, monitoring vital signs, and providing therapeutic interventions as outlined in care plans. * Assessment and Monitoring: Conduct comprehensive assessments of patients' mental health status, patients' medical status, track progress, and report any significant changes to the supervising psychiatrist or healthcare team. * Care Coordination: Collaborate with healthcare professionals, including psychiatrists, social workers, and care managers, to develop and implement individualized care plans. * Education and Support: Provide education to patients and their families about psychiatric conditions, medical conditions, treatment options, and medication management. Offer support and guidance to help patients navigate their mental and physical health journey. * Documentation: Maintain accurate and timely documentation of patient interactions, care provided, and any changes in patient status. Ensure all records comply with relevant regulations and standards. * Crisis Intervention: Respond to emergency situations with appropriate interventions and support, coordinating with crisis teams as needed. * Compliance: Adhere to all regulatory and agency standards, including safety protocols, confidentiality requirements, and infection control procedures. * Community Engagement: Participate in community outreach and education efforts to promote mental health awareness and reduce stigma associated with psychiatric conditions. WHAT WE ARE SEEKING: Qualifications: * Education: Completion of a Practical Nursing program and possession of a valid LPN license. * Experience: At least five years of experience as an LPN. Previous experience in psychiatric or mental health nursing is highly preferred. Experience in a field-based or community health role is a plus. * Skills: Strong clinical assessment skills, excellent communication abilities, and proficiency in electronic health records. Ability to work independently and manage time effectively in diverse and sometimes challenging environments. * Certifications: Basic Life Support (BLS) certification required. Additional certifications in psychiatric nursing or mental health care are advantageous. * Driving: Valid driver's license and reliable transportation required, as travel to patient locations is an essential part of the role. This is a field-based role with a coverage radius of 30+ miles. Skills: * Compassionate and patient-focused. * Proactive in identifying and addressing community needs. * Skilled in adapting to evolving technology and processes. * Excellent organizational and problem-solving abilities. * Familiarity with Google Suite (Gmail, Google Docs, etc.). Able to quickly learn and effectively use new healthcare-related software. * Phlebotomy skills. * Administration of medications, including intramuscular (IM) injections.
