Position Summary Program Acquisition Coordinator - Hospital Care at Home, ORMC NURSING - Downtown Orlando - Hospital Care at Home - Orange County - Day Location: Orange County, FL Position Type: Full-Time (Day Shift) 7:30 - 5:30 Rotating Holidays and Weekends Note: This is NOT a remote/work-from-home position. You will be required to travel to patients' homes to provide in-home acute nursing care and work in the patient care hub to provide virtual care. About the Hospital Care at Home Program: The Orlando Health Hospital Care at Home program delivers acute hospital-level care to adult patients within the comfort of their own homes. This role provides nurses with the unique opportunity to build strong, personal connections with patients and their families. Our enhanced acute-care model utilizes portable technology to connect patients to the Orlando Health Patient Care Hub for 24/7 remote monitoring and virtual care. Position Summary: The Program Acquiition Coordinator leads the patient acquisition efforts identifying referral sources, patient selection, reviews referrals, and coordinates admissions to the Hospital Care at Home unit. Education/Training: Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program Licensure/Certification: Current licensure as a registered nurse in the State of Florida or Nurse Licensure Compact (NLC) or as a licensed practical nurse in the State of Florida. Experience: One (1) year experience in the health care field.
