RN Care Manager Transitions of Care


The Triad Healthcare Network (THN) Hospital Liaison Nurse delivers in-patient care services to a contracted community based high-risk population. Working under close supervision, this role communicates effectively regarding the clinical and psychosocial patient information with the healthcare team for case management. Talent Pool: Nursing Talent Pool: Nursing Orchestrates discharge planning of Care Management patients that are exiting the facility to ensure a safe and prompt release. Completes follow-up for discharged inpatients with complex medical and psychosocial issues to evaluate health standing. Documents information in the intake process in the Care Management electronic documentation system for accurate record keeping. Educates healthcare providers and patients about the benefits of the Care Management program to ensure the utilization of its services. Screens and assesses patients to understand health conditions and effectively monitor progress until the eventual discharge back into the community. Performs other duties as assigned. EDUCATION:Required: Associate's Degree Preferred: Bachelor's Degree EXPERIENCE:Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations LICENSURE/CERTIFICATION/REGISTRY/LISTING:Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider Equal Opportunity Employer At Cone Health, we strive to create a welcoming atmosphere that celebrates a diverse and unique workforce.
