Public Health Nurse II - Part Time
Gaston County Public Health is committed to protecting, promoting and preserving the health and well-being of all Gaston County residents. The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform. * Clinic Duties: Utilizes the Nursing Process to assess, interview, triage, educate and implement the plan of care. Examples: complete vital signs, interviews, and patient / family education. Administers immunizations per CDC guidelines and documents in NCIR. Provides comprehensive immunization services for individuals, families and the community. Delivers services through clinics, home visits, case management, mobile units, and community outreach programs. * Leadership: Act as Back-up to the Immunization Coordinator. Is able to run reports from NCIR, assists with correcting errors in NCIR, adheres to vaccine storage and handling requirements, receives and documents vaccine shipments, monitors inventory, coordinates seasonal vaccine clinics, mentors and serves as a resource for all staff and is able to work independently to ensure community needs are met as it relates to Immunizations. * Assigned Follow-Up: Nursing team members will be assigned specific tasks by Nursing Supervisor, Charge Nurse, or Immunization PHNIII to assure that contract addenda requirements and clinic specific needs are met. Examples: AED and Code Cart monitoring, Immunization refrigerator temperature monitoring, follow-up of missed appointments, contacting patients that are due for vaccines, and other miscellaneous tasks as needed. * Quality Assurance/ Quality Improvement: Complete monthly audits and per agency schedule to assure compliance with standing orders and ACIP guidance. Discuss audit findings with members of the team as an educational and quality improvement opportunity. * Other Duties As Assigned: Attends all mandatory agency meetings and trainings; Assists the community and county schools trainings related to emergencies such as natural disasters or bio-terrorism events by delivering services in the field as needed. Participates in staffing emergency shelter, administering vaccinations to the community and providing nursing care to assure community safety. * Carryout Performance Management (PM) roles and responsibilities, including identifying and communicating performance improvement opportunities and adjusting performance and practice to achieve established PM goals. * Implement strategies aimed at achieving Health Equity, including but not limited to: attending trainings to better understand inequities, identifying and addressing barriers to health, serving on the Health Equity Team or its subcommittees, engaging in internal or external efforts to eliminate health disparities. Graduation from an accredited school with a Bachelor's Degree in Nursing which includes a public health rotation OR graduation from an accredited school of Nursing and one year of public health experience. A combination of experience and education will be considered. For Associates Degree or Diploma in Nursing, one year of professional nursing experience required. Must possess a current RN license valid for practice in the state of North Carolina, a valid BCLS/CPR Certification, and a valid drivers license. A copy of your RN license and CPR certification must be attached to your application.
