Dir Case Management

Overview Provides administrative structure and support to manage and direct the services provided in the Case Management Department. Accountable for strategic planning, implementation, evaluation, and revision of systems to support the department's goals, and the organizational goals. The Director of Case Management provides leadership to achieve the organization's mission of excellent patient care, and the departments focus on assisting the patient/client in navigating through and transitioning out of the health care system. Qualifications Minimum Education and Experience Requirement: Current RN license in state of Florida. Master's degree in Nursing required. Consideration will be given to those currently enrolled in formal education to obtain MSN. National nursing certification required within six months of hire. Certification in Case Management is required within six months of hire. Minimum of five years professional nursing practice experience required. Demonstrated leadership and management skills with knowledge and application of case management, care delivery models, health care delivery systems, CQI process, and budgetary process. Must be able to demonstrate skills in creating vision, effecting change and facilitating transitions. Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose as a 'frequent driver' NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
