Purpose of Job: Improve the overall health of the communities we serve by supporting the CHAS Health case management team tasked with caring for patients with complex needs. Essential Duties and Responsibilities: * Outreaches to and engages with patients needing additional support to improve their health outcomes. * Conducts intakes and assessments of patient needs. Provides comprehensive and ongoing identification of needs such as health care, social and education. Advocates and refers patients to appropriate resources. * Assesses and assists with non-clinical problems interfering with effective patient care. Documents progress and follows up. * Works closely with Case Managers and care teams to develop and implement care plans for identified patients. * Counsels and aids patients and their families to understand health care recommendations and options. * Coordinates with outside agencies to ensure patients are given all resources to attend scheduled appointments. * Completes required reporting for disease state if applicable. * Maintains tracking and appropriate use of CHAS supplied resources. * Performs other duties assigned, including supporting the CHAS Health Mission and Core Values. Hospital-Based Care Coordinators will spend 90% of time or more at local emergency departments, which may include evenings and weekends. Additional background and testing may be required based on hospital requirements. Outpatient Care Coordinators will spend majority of time in community settings, including CHAS Health clinics. Medical Assistant will assist Triage RN as needed and outreach to patients recently discharged from hospitals. Qualifications: Education/Experience: Washington Locations: One year of health and/or social services experience required. Valid driver's license and insurance required. Completion of Community Health Worker training through the State of Washington required. Completion of medical assistant training through an apprenticeship program registered and approved by the WA Department of Labor & Industries or college program accredited by the Accrediting Bureau of Health Education School (ABHES) or the Commission of Accreditation of Allied Health Education Programs (CAAHEP) required. Certification through the American Association of Medical Assistants (AAMA), National Healthcareer Association (NHA) or National Center for Competency Testing (NCCT) required within 120 days of hire. Washington locations also require a Medical Assistant - Certified credential issued by Washington State Department of Health. Previous experience in a community health clinic preferred. Experience with EHR preferred. Idaho Locations: One year of health and/or social services experience required. Medical Assistant - Registered credential issued by Washington State Department of Health within 120 days of hire. Certification through the American Association of Medical Assistants (AAMA), Certified Clinical Medical Assistants (CCMA) or National Center for Competency Testing (NCCT) required within 120 days of hire. Previous experience in a community health clinic preferred. Experience with EHR preferred. Skills: Excellent interpersonal, written and verbal communication skills, with an ability to work effectively with wide range of people. Must be able to work well independently and with a team. Excellent analytical skills required. Motivational interview skills preferred. Computer skills required. BLS (CPR/AED) required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required. Physical Demands: Required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day. Communicating by talking / hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
