Pediatric Home Health Case Manager TX (2024)
Amazing Care Pediatric Home Health is seeking a dedicated and compassionate Pediatric RN Case Manager to join our team in the Corpus Christi area. As a vital member of our healthcare team, you will be responsible for providing comprehensive case management services to pediatric patients and their families. This role involves coordinating care, advocating for patients, collaborating with healthcare providers, and ensuring that patients receive the highest quality of care. Responsibilities: * Conduct initial assessments of pediatric patients to determine their healthcare needs and develop individualized care plans. * Coordinate and monitor patient care to ensure timely and appropriate services are provided. * Collaborate with healthcare providers, including physicians, therapists, and specialists, to facilitate continuity of care. * Advocate for patients and families to ensure their needs are met and their voices are heard in the healthcare system. * Educate patients and families about their healthcare options, treatment plans, and resources available to them. * Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress notes. * Participate in multidisciplinary team meetings to discuss patient cases and develop comprehensive care plans. * Qualifications: * Registered Nurse (RN) license in the state of Texas. * Bachelor of Science in Nursing (BSN) preferred. * Minimum of 1 years of experience in pediatric nursing or case management. * Strong clinical assessment and critical thinking skills. * Excellent communication and interpersonal skills. * Ability to work independently and as part of a multidisciplinary team. * Knowledge of community resources and healthcare services available to pediatric patients and families. * Commitment to providing compassionate care and advocating for the well-being of pediatric patients. Compensation: $60,000-$70,000 annually Benefits: Medical, Dental, Vision, 401(k), PTO, Short Term Disability, Long Term Disability and many more.
