Registered Dietitian, Certified Diabetes Educator (Outpatient), Clackamas, Full-time

Job Summary: Provides diabetes self-management education and training to patients with diabetes and their family members. Coordinates and monitors outpatient nutrition counseling services provided to outpatients/clients served including nutrition assessment and screening, development of nutrition and diet plans and counseling. Completes progress notes, serves as a liaison and provides documentation in medical record to client's personal physician. Job Requirements: Education and Work Experience: * Bachelor's Degree in food and nutrition or equivalent (some certifications will require Bachelor's): Required * Master's Degree: Preferred * Experience as a Clinical Dietitian: Preferred Licenses/Certifications: * Registered Dietitian (RD): Required * Certified Diabetes Educator (CDE): Required Essential Functions: * Provides diabetes self-management education and training to patients with diabetes and their family members. Provides individualized or group, goal directed diabetes management education for patients and their families according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators. Provides diabetes education to patients, caregivers, staff, and the community. Acts as an educational resource for health care team and others including dietetic interns, patients, family members and community. * Works collaboratively with multi-disciplinary diabetes care team to implement plan of care. Participates in the development of training materials. Provides care to patients suffering from acute or chronic hyper/hypo glucose management, managing therapeutic regimes according to protocols. Provides educational interventions that utilize mainly interactive, collaborative, skill-based training methods and maximizes interactive training methods. Works on problems of diverse scope and complexity. Exercises independent judgment on moderately complex cases. * Develops a Plan of Care that will include, but is not limited to, self-care skills, disease process, medication, diet, activity, home safety management and emergency procedures. Provides Medical Nutrition Therapy to outpatients with diagnoses other than diabetes. * Performs nutrition assessment, reassessment and patient/family education. Performs all responsibilities/duties required by Registered Dietitian (RD) as defined in the scope of practice, to ensure the needs of patients are addressed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision-making skills. Follows American Dietetics Association (ADA) Medical Nutrition Therapy and other appropriate nutrition counseling protocols. Completes required documentation and charting.
