COMMUNITY CARE PRACTITIONER CLASSIFICATION TITLE: COMMUNITY CARE PRACTITIONER (Non-Physician) PURPOSE OF CLASSIFICATION The purpose of this classification is to provide scheduled and unscheduled direct patient care in a mobile environment, to perform technical and specialized work functions associated with providing professional, independent medical care to non emergent and emergent patients, will work collaboratively with other members of the healthcare team to provide assessment, diagnosis, and treatment of individuals of all age groups with acute or sub-acute injuries or illnesses. Provides assessment and treatment within scope of practice and reports to The Department Medical Director concerning clinical care matters. Reports to the Fire/Chief Operating Officer concerning matters of employment, programs, and/or department goals/protocols. ESSENTIAL FUNCTIONS The following duties are normal for this position. The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned. Delivers a high quality frontline mobile integrated healthcare services including but not limited to scheduled and unscheduled in home visits, acute care, health risk assessments, transitional care, longitudinal high risk patient management, advanced injury and illness management, adjunctive care for home health, hospice, and primary care services. Provides a comprehensive physical examination and assessment and consults with the physician as needed to develop the treatment plan and/or implementing physician directives. Orders laboratory, radiology, and other diagnostic testing as appropriate and within scope of practice and/or with physician consultation. Interprets diagnostic test results for deviations from normal. Obtains, compiles, and records patient medical data, including health history, progress notes and results of physical examination. Prescribe medication as approved by The Department Medical Director. Responsible for accurately coding all encounters and procedures in accordance with Medicare Rules and Regulations. May facilitate referrals of patients to other medical providers, medical facilities, or other health/social service agencies when appropriate as per the delegation of the supervising physician. In a life threatening emergency situation, when the supervising physician is not present, the Community Care Practitioner may initiate the appropriate evaluation and treatment. Ensures medical readiness of unit; responds to emergency and non-emergency medical calls; assesses and determines needs of the patient and/or situation; communicates information to the Emergency Medical Dispatcher. Assigns priorities of care; assesses critically ill and injured patients and determines proper treatment. Assesses the effects of treatment; relays complete medical information to hospital personnel assuming the case; provides Emergency Medical Transport of injured persons as necessary. Receives, verifies, and responds to emergency and non emergency calls; evaluates scene upon arrival; may direct traffic and control crowds. Prepares for and conducts training and instruction in procedures, emergency medicine; may prepare lesson plans, handouts, and audio/video/teaching aids. Monitors and communicates effectively and coherently over radio channels while initiating and responding to radio communications. Supervises and prepares, inspects, repairs, and/or maintains Emergency Medical Equipment and vehicles for safe operation; tests equipment on a regular basis. Establishes and maintains good working relations with the public; attends community meetings; promotes and teaches injury prevention; responds to questions and requests for information. Inventories supplies and equipment; orders and maintains sufficient amount to ensure adequate units are available to perform daily tasks. Communicates with supervisor, employees, other departments, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems. Completes and prepares a variety of forms, logs, requests, records, reports, correspondence, and various other documents associated with daily responsibilities of this position; maintains patient records; maintains administrative records and files. Maintains current filed and code manuals, policies and procedures, employee handbooks, various maps, addresses, and related material for reference and/or review. Maintains a comprehensive, current knowledge and awareness of applicable laws/regulations and current medical standards/techniques; reads professional literature; maintains professional affiliations. Attends meetings, seminars, drills, workshops, and training sessions as appropriate to maintain knowledge of departmental and County Operations, to promote improved job performance, and to stay current with medical standards/techniques; may serve on panels, boards, and/or committees. Cooperates with federal, state, and local law enforcement agencies and their officers or representatives when activities are related to investigations within County jurisdiction. ADDITIONAL FUNCTIONS Performs miscellaneous duties at assigned station. Answers the telephone; provides information, advice and guidance; takes and relays messages and/or direct calls to appropriate personnel; returns calls as necessary. Substitutes for co-workers in temporary absence of same; provides assistance to other employees or departments as needed. Performs other related duties as required. MINIMUM QUALIFICATIONS Master's degree in Nursing. Must possess and maintain a Georgia License as a Registered Professional Nurse (RN) and certification as a Nurse Practitioner (NP)/Advanced Practice Registered Nurse (APRN) with the Georgia Board of Nursing; OR Bachelor's degree required, with a Master's degree preferred, from the successful completion of a Physician Assistant (PA) Program from an accredited college or university. Must possess and maintain a Physician Assistant Certification (PA-C) by passing the Physician Assistant National Certifying Exam administered by the National Commission on Certification of Physician Assistants (NCCPA). ADDITIONAL QUALIFICATIONS Must be a US Citizen or resident status and must possess and maintain a valid Georgia driver's license. Must possess and maintain all required State of Georgia certifications and/or licensures. Must qualify for the Centers for Medicare & Medicaid Services National Provider Identifier (CMS NPI) Standard. Must possess and maintain the basic Cardiopulmonary Resuscitation (CPR) and the Advanced Cardiovascular Life Support (ACLS) certifications. Must be able and willing to provide service in the home and in a non-clinic environment. Emergency Room (ER) experience preferred. May be required to attain and maintain additional certifications specific to assigned unit.
