Posting Summary Rutgers, The State University of New Jersey, is seeking a Nursing Care Coordinator for NJMS- Ambulatory Care/Infectious Diseases. The primary purpose of the Nursing Care Coordinator position is to coordinate and deliver specialized nursing care services. Working within a healthcare setting, you will be responsible for ensuring that patients receive timely, compassionate, and evidence-based care, while also addressing their unique physical, emotional, and social needs. This role involves collaboration with a multidisciplinary team to optimize patient outcomes and enhance the overall quality of care Among the key duties of this position are the following: * Performs triage and medical assessments within scope of practice. * Conducts thorough assessments of patients living with and/or at risk for HIV/AIDS to determine their healthcare needs. * Coordinates and monitors the delivery of medical treatments, medications, and interventions. * Monitors and tracks patient progress, ensuring adherence to prescribed treatment plans. * Provides education and counseling to patients and their families regarding HIV prevention, treatment options, and adherence to care plans. * Offers counseling and emotional support to patients and their families, addressing psychological and social aspects of living with HIV. Seniority Unit Terms of Appointment Staff - 12 month Position Pension Eligibility ABP Qualifications Minimum Education and Experience * Graduate of an accredited school of nursing with a license to practice in the State of New Jersey. * Registered Nurse Licensure in the State of New Jersey. * Prior experience: At least two (2) years of outpatient experience and/or at least 3 years of relevant experience. * Two (2) years of clinical nursing experience with familiarity in the use of electronic medical records (EMR). Certifications/Licenses Required Knowledge, Skills, and Abilities Preferred Qualifications * Bachelor of Science in Nursing preferred. * Experience in Infectious Diseases, HIV/AIDS care is preferred. Equipment Utilized Physical Demands and Work Environment Physical Demands: Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting 1/3 of the time, use of hands to finger, handle, or feel over 2/3 of the time, reach with hands and arms 1/3 of the time, stoop, kneel, crouch, or crawl 1/3 of the time, talk or hear over 2/3 of the time. Requires lifting of up 2/3 of the time. Occasionally requires ability to push wheeled carts/stretchers more than 100 pounds up to 1/3 of the time.
