Registered Nurse
Overview: The Registered Nurse (RN) is a vital member of the home health care team, providing skilled nursing care to patients in their homes. The RN assesses patients' needs, develops and implements individualized care plans, and collaborates with other healthcare professionals to ensure optimal patient outcomes. Responsibilities: * Conduct comprehensive nursing assessments of patients to determine their physical, emotional, and psychosocial needs. * Develop and implement individualized care plans based on assessment findings, medical orders, and patient goals. * Administer medications and treatments as prescribed by physicians, ensuring accurate documentation and adherence to nursing standards of practice. * Provide wound care, ostomy care, and other complex nursing procedures as needed. * Educate patients and caregivers about disease management, medication administration, and self-care techniques. * Monitor patients' condition and report any changes in status to the physician and other members of the healthcare team. * Collaborate with other healthcare professionals, including physicians, physical therapists, occupational therapists, and social workers, to coordinate patient care. * Maintain accurate and up-to-date patient records, including progress notes, care plans, and medication administration records. * Adhere to all agency policies and procedures, as well as state and federal regulations governing home health care. * Participate in quality improvement initiatives and continuing education programs. Qualifications: * Graduate of an accredited nursing program. * Current and valid Registered Nurse (RN) license in the state of practice. * Previous experience in home health care preferred. * Strong clinical assessment, problem-solving, and decision-making skills. * Excellent communication and interpersonal skills. * Ability to work independently and as part of a team. * Proficiency in electronic health records (EHR) systems. * Valid driver's license and reliable transportation.
