The Population Health Care Supervisor, under the direction of the Manager of Population Health, has primary responsibility for care management activities for a specific patient panel. This will include identifying the high acuity patient population and developing, conducting, and monitoring care management processes. In addition, maintains responsibility for supervising assigned staff. REQUIRED * Bachelor of Science Degree in Nursing and a minimum of 3 years of experience in a clinical setting OR Associates Degree in Nursing with a Bachelor of Science Degree in a health related field with a minimum of 5 years' experience in a clinical setting * Proficient computer skills, including Microsoft Office (specifically Word and Excel) * Excellent oral and written communication skills. * Good organizational, analytical, and problem-solving skills. * Ability to function effectively with a wide variety of disciplines, including physicians and nurses. * A team player that can follow a system and protocol to achieve a common goal. * Confidence to communicate and collaborate with other community health care organizations and personnel. * Able to participate as part of a team to accomplish an assigned project. * Demonstrated ability to set priorities and make appropriate clinical and supervisory decisions. Must possess good problem solving skills. PREFERRED * Previous experience in a Primary Care Setting. * Previous experience in case management, care coordination, or patient navigation. * Previous population health experience.
