Our Expert Care. The job you have been looking for! AuthoraCare Collective is currently seeking a CAP RN Case Manager for our Greensboro service areas. This position is hybrid with flexible hours. Job Summary: Manage the over-all plan of care for a minimum case load assignment of CAP-C and CAP-DA recipients as determined by the Director of Kids Path. Assess and identify recipient/caregiver problems, establish goals, and interventions. Monthly recipient contacts for assessment of ongoing needs, interventions, and service utilization. Oversee clinical needs for patients , in partnership with social work case managers. Nursing/case management skills needed include excellent recipient needs assessment and strong psychosocial/familial assessment skills, critical thinking, problem solving, delegation, effective interpersonal and communication skills necessary. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Directs and coordinates all services provided through CAP to recipients in accordance with the recipient's individualized care plan. Utilizes case management process in planning and prioritizing recipient care needs, including assessment and identification of recipient/caregiver problems; identification and implementation of interventions; and evaluation of outcomes. Partners with the assigned social work case manager to complete annual reassessment process. Required Qualifications: * Graduate of an accredited School of Nursing * ADN or BSN required, active registered nurse license in North Carolina * One year experience in nursing required * Additional two years of case management experience preferred * Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
